HHI helps community members find the next step when they need medical, social, or insurance support. The referral service connects participants to free and low-cost resources in the community, including screenings, Federally Qualified Health Centers, medication assistance, ACCESS, and Health Insurance Marketplace navigation.
Applications, eligibility rules, appointment systems, and insurance language can be difficult to manage alone. HHI provides bilingual support so participants can understand options and prepare the information needed to move forward.
Common referrals include mammograms, PAP tests, clinics, medication programs, public benefits support, and Marketplace navigators. HHI does not replace a medical provider; it helps people reach the appropriate services more quickly.
