HHI's Health Risk Assessment service helps uninsured and under-insured adults understand their current health risks before a preventable condition becomes a crisis. Community Health Workers provide bilingual, culturally responsive screening, explain results in plain language, and connect each participant with education, follow-up coaching, and referrals when results show a need for additional care.

What participants receive

Screenings may include body mass index, blood glucose, A1C, blood pressure, pulse, cholesterol, and vision checks. The visit is designed to be practical and respectful: participants update their health status, discuss personal and family health history, and leave with next steps they can understand and act on.

Follow-up support

When a screening points to elevated risk, HHI invites participants into one-on-one case management and coaching with a Community Health Worker. That support can include goal setting, nutrition education, reminders, referrals to free or low-cost care, and encouragement as participants build healthier routines.
