What participants receive
Screenings may include body mass index, blood glucose, A1C, blood pressure, pulse, cholesterol, and vision checks. The visit is designed to be practical and respectful: participants update their health status, discuss personal and family health history, and leave with next steps they can understand and act on.
Follow-up support
When a screening points to elevated risk, HHI invites participants into one-on-one case management and coaching with a Community Health Worker. That support can include goal setting, nutrition education, reminders, referrals to free or low-cost care, and encouragement as participants build healthier routines.
