“Taking Care of My Health”/“Cuidando Mi Salud”

Health Risk Assessment Program
The Health Risk Assessment program was designed by HHI to promote wellness and improve health indicators among uninsured and under-insured adults at risk of chronic disease, by providing community based, culturally- and linguistically competent health screening, education, and case management. The program’s goal is to improve participants’ health risk profiles as determined by:

  • Weight (Body Mass Index)
  • Blood glucose
  • A1C
  • Blood pressure
  • Pulse
  • Cholesterol
  • Vision

At the TCMH/CMS health risk assessment, HHI’s Community Health Workers conduct health screenings for participants, to update their health status. Participants then receive one-on-one health education, based off the results of their screens and their personal and familial health history, to inform them of their risk factors for chronic disease. Finally, participants are invited to take part in a one-on-one case management/coaching follow-up with the CHW, to support and promote healthier lifestyles.

A Cancer Support Group, for Hispanic men and women, who are: patients, survivors and/or caregivers, living with any type of cancer.

  • Federally Qualified Health Centers
  • Free/Low Cost Medication programs
  • ACCESS
  • Health Insurance Marketplace Navigators
